Community Navigators

How to access the service

Community Navigators will be based across the Wokingham Borough and will be able to help you find out what is on in your community.

If you want to refer someone, or want someone to contact you to help you navigate support, charities and community groups available in your area then you can complete an online referral form or download the referral form. Please complete and return it to the Community Navigator Coordinator by post or by email.

Forms or referrals that are received by involve will be allocated to your local Navigator who will contact you to arrange a meeting at a local venue or they may discuss with you what you need over the telephone or by email.

You may also be referred by your GP. Some of the GP surgeries across Wokingham Borough will have Navigators in the surgery. If this is the case, your GP will complete the referral form with you, and at the Reception of your surgery you will be able to make an appointment to see a Community Navigator at the next available time.

The Community Navigators will find out what is available to meet your needs, not to advise you what to do; it's your choice but we are here to help.  However, if you are worried about taking the next steps then in some circumstances we may be able to make telephone calls for you on your behalf. It is our aim to connect you with the services and activities that will let you live a happier, healthier and independent life.

All Community Navigators have been police-checked, trained and can help and guide you through what services and activities are available in your area. Navigators will generally be using WIN (the Wokingham Information Network), a fantastic resource. You may find everything you need here and not need a Navigator to assist you. But sometimes things can become much clearer when you have someone to assist you to explore what you need - over a cup of tea!
